Customized to meet the needs of each community, this pathway utilizes a holistic, partnered approach for treating memory care residents, by providing comprehensive, interdisciplinary treatment for seniors living with dementia to improve their quality of life and daily activities performance. Emphasis is placed on activities that enhance physical, emotional, spiritual, occupational, intellectual, social health, and well-being. Comprehensive standardized assessments are used to determine the optimal approach to use with each individual. Application of evidence-based practices maximize function through nursing and rehabilitation services. Modification of the environment, connects the individual’s habits and routines to activities of daily living.
