As we reflect on an extraordinary 2025, we are proud to recognize three exceptional leaders whose dedication, consistency, and vision helped elevate performance across multiple regions and communities throughout the country.
The North Star Award recognizes the facility team that demonstrates the strongest overall performance across key operational and clinical indicators. Evaluation criteria include percentage margin over budget, audit performance, home exercise program participation, program tag engagement, and Core4 participation.
This year’s North Star Award was presented to Hannah Gibson, Director of Rehabilitation, and her outstanding team at Givens Highland Farms.
Under Hannah’s leadership, her team delivered exceptional results across every major performance category, establishing her facility as a model for clinical excellence and operational consistency. But Hannah’s impact extends far beyond her own community.
Throughout the year, Hannah has continued to invest in the growth and development of fellow clinicians across Functional Pathways by supporting our Clinical Fellowship Year calls and contributing to our speech-language pathology mentorship initiatives. Her willingness to lead, teach, and serve others reflects the type of leadership that strengthens not only individual communities, but our organization as a whole.
Hannah’s passion for people, commitment to quality, and dedication to mentoring future leaders make her a true North Star for Functional Pathways.
The Momentum Award recognizes the Area Director of Operations region demonstrating the greatest year-over-year improvement across both operational and clinical performance metrics, including margin performance, audit scores, home exercise program participation, program tags, and Core4 engagement.
This year’s Momentum Award was presented to Luis Estrella.
What makes this achievement even more remarkable is that Luis also earned this award last year.
In healthcare operations, maintaining momentum is challenging. Building upon success year after year is even more impressive.
Luis and his region once again demonstrated what intentional leadership, strategic execution, and team alignment can accomplish. The Light-Year Award recognizes the Area Director of Operations region with the strongest overall standing in both operational and clinical performance.
This year’s Light-Year Award was presented to Ty Manring.
Ty’s leadership style is built on one of the most important drivers of long-term success in senior living and rehabilitation services: relationships.
