Peak Senior Living offers Holistic Therapy and Wellness for Independent, Assisted Living, CCRC and Memory Care Communities.

Functional Pathways, a full-service contract therapy provider since 1995, recently rebranded its senior living division. Functional Pathways Preferred (FPP) was relaunched as PEAK Senior Living to put more focus on its integrated approach to person-centric wellness. PEAK offers outcome-based solutions that engage and empower seniors to enhance social, emotional, and physical wellbeing.

According to the Centers for Disease Control and Prevention (CDC), more than one in four older adults report a fall each year. That’s over 3 million falls per year. In 2020, 36,508 adults aged 65 and older died from preventable falls. For many, the fear of having a serious injury that would end their independence is a stress-inducing reality. PEAK’s outcome-based therapy and wellness programs have been shown to reduce falls with major injury by 45%.

PEAK’s programs support the Seven Dimensions of Wellness: Physical, Emotional, Intellectual, Social, Spiritual, Environmental, and Occupational. Programs help with balance, cardiovascular functioning, mobility, muscle strength, and even cognition, to reduce the negative impacts of aging. Resident health is consistently supported so they move seamlessly through the continuum of care between leisure, wellness, rehabilitative therapy, home health and back. By giving communities and residents the right tools to stay healthy, active, and engaged at every stage of wellbeing, seniors can age in place gracefully without the fear of becoming injured and having to leave home.

“Our goal is to ensure that residents always have the right therapy or wellness solution at the right time, no matter where they are on their wellness journey, to reduce the risk of hospitalization,” said PEAK Divisional Vice President, Kathryn Wyatt. “We play a pivotal role in helping residents stay healthy and independent for as long as possible.”

One NEW feature of PEAK’s services is the addition of an Active Lifestyles Navigator, or resident health liaison, who regularly assesses each resident for improvement or decline. Regular Fit Tests help determine where residents are on the wellness spectrum, so they can be referred to the right program or therapy. Progress is measured and tracked for improvement or decline.

Turn-key services offer assisted living facilities and senior communities a holistic, systematic approach to improving resident health and independence. PEAK’s therapeutic outcomes have exceeded national averages, with 94% of orthopedic patients meeting therapy goals and 86% of patients treated for back pain seeing improvement. The goal is to meet seniors wherever they are on their personal wellness journey and deliver the right therapy and wellness tools at the right time, so communities can keep residents engaged and thriving in place longer.
